An unannounced SHSI inspection was conducted today from 11:30 am-1:15 pm. There were no children on site at the time of the inspection. Vendor/ household/ child information was reviewed. Emergency planning and supplies were also inspected. Transportation permission and requirements were discussed. Hand washing and cleaning routines of children in care were also reviewed/ discussed. Standard #: 22VAC40-665-170-B Description: Based on information reviewed, required documentation for child records was incomplete. Evidence: Child A's record missing emergency contact address, telephone number name. Child B's record missing parent work address, telephone number and emergency contact name and address. Child C's record missing parent work address, telephone number and emergency contact name and address. Child D's record missing parent work address, telephone number and emergency contact name and address. Plan of Correction: I will get all missing information. Vendor spoke with parent during inspection.
Standard #: 22VAC40-665-210-B Description: Based on record review, required child immunization records due at time of enrollment were not on site. Evidence Child A start date 3/25/17 has no immunization record . Child B start date 10/7/19 has no immunization record. Child C start date 10/7/19 has no immunization record. Child Dstart date 10/7/19 has no immunization record. Plan of Correction: I have asked parent for 3 days ago. Called during inspection to get from school.
Standard #: 22VAC40-665-390-A Description: Based on observation, required emergency supplies are incomplete. Evidence: There was no digital thermometer in emergency supplies. Plan of Correction: I will get.
