Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A monitoring inspection was initiated on 12/14/2021 and concluded on 12/17/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 40 children present, ranging in ages from 5 months to 5 years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

New requirements became effective on 10/13/21. The facility has not fully complied with the following requirements:

8VAC20-780-350- F The center shall develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care byconsistent staff or team of staff members.

8VAC20-780-550 ? Procedures for emergencies / Lockdown / Documentation of Drills

The center is using the information from the model form Staff Orientation Documentation to ensure all staff orientation training requirements are being met.

The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on record reviews, the center failed to obtain central registry findings within 30 days of employment.
Evidence:
1. The inspector reviewed four staff records. Two of four records reviewed (staff records #1 and #2) did not contain findings from the search of the central registry. Staff #1's and staff #2's date of employment was 07/04/2021.

Plan of Correction: The search of the central registry requests have been completed and submitted. As soon as the results are obtained, copies will be sent to the licensing office.

Standard #: 8VAC20-780-130-A
Description: Based on record reviews, the center failed to obtain documentation of child's immunizatoins prior to attendance.
Evidence:
1. The inspector reviewed three children's records. One of three records reviewed (child record #3) did not contain documentation of immunizations. Child's first day of attendance was 11/07/2021.

Plan of Correction: The center has requested a copy of the child's immunization documentation. Once the immunization document is obtained, a copy will be sent to the licensing office.

Standard #: 8VAC20-780-140-A
Description: Based on record reviews, the center failed to obtain documentation of child's physical examination prior to child's attendance or within 30 days after the first day of child's attendance.
Evidence:
1. The inspector reviewed three children's records. One of three records reviewed (child record #3) did not contain documentation of a physical examination. Child's first day of attendance was 11/07/2021.

Plan of Correction: The center has requested a copy of the child's physical examination. Once the copy of the child's physical is obtained, a copy will be sent to the licensing office.
