Comments:
A renewal inspection was initiated on December 16, 2021 and concluded on December 20, 2021. There were twenty-nine children present, ranging in ages from infants to five years, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observation, facility did not ensure there is adequate fall zones and resilient surfacing under and around climbing equipment and/or equipment with moving parts on the outside playground.

Evidence: The following was observed on outside playground:
1. There was approximately 3' between the tunnel train and arch climber where there should be at least 6'.
2. There was no resilient surfacing under and around the arch climber and tunnel train, both of which are pieces of climbing equipment.

Plan of Correction: Resilient surfacing will be purchased and put under and around all pieces of climbing equipment. Equipment will be moved on the playground to ensure there is at least 6' around all pieces of climbing equipment.
