This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documentation review, interview, and a virtual tour of the program.
A subsidy health and safety inspection (SHSI) was initiated on 12/2/21 and concluded on 12/9/21. There were 16 children present with two staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-665-520-B-11 Description: Based on documentation review and interview, the center did not obtain written allergy care plans for each child with a diagnosed food allergy to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The record for child A included documentation that the child was allergic to avocado, banana, and blackberries. The record did not contain an allergy care plan completed by a physician. 2. The allergy list submitted by the center included documentation that Child B was allergic to mangoes. The center did not submit an allergy care plan for Child B completed by a physician. Plan of Correction: All children with allergies will have the proper documents completed and viewed by Daycare Director, prior to enrollment.
