Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on December 13, 2021 and concluded on December 14, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 150 children present, ranging in ages from two months to six years of age, with 31 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records, seven full staff records, and 14 staff records with only the tuberculosis documentation and background checks reviewed.
New requirements became effective October 13, 2021. The facility has not yet fully complied with the following requirements: Written shaken baby syndrome and abusive head trauma policy, and new requirements for children with allergies.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-70
Description: Based on review of staff records, the center failed to have documentation of required tuberculosis screening within the required time frame for those employees hired before 10/13/2021.

Evidence:
1. Staff #17's tuberculosis screening on file was dated 08/26/2021. Date of employment for Staff #17 was 07/29/2021. The tuberculosis screen should have been completed by 08/19/2021.
2. Staff verified the date of employment for Staff #1, and the date on the tuberculosis screening on file.

Plan of Correction: There was a change in administration that handles staff paperwork and onboarding, and the tuberculosis screening was overlooked. Staff are now required to get their tuberculosis screening at hire before beginning in a classroom.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that all hazardous substances were kept in a locked location.

Evidence:
1. In Classroom #5 the bottle of bleach water was hooked on the side of the changing table.
2. Staff verified that the bottle of bleach was not in a locked location.

Plan of Correction: Staff immediately moved the bottle of bleach water to a locked location. Administration will review hazardous materials standards with staff and do drop in observations in the classrooms to ensure hazardous materials are kept in a locked location.
