Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A subsidy health and safety (SHSI) inspection was initiated on December 15, 2021 and concluded on December 15, 2021. There were 82 children present with 13 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-665-580-B
Description: Based on record review and interview, the vendor did not ensure within the first 90 days of employment of service all caregivers complete the Virginia Pre-Service Training for childcare staff.
Evidence: 1. The record of staff #2 (DOH 7/29/21) did not contain documentation of Pre-Service training. 2. Administration acknowledged the center did not have it.

Plan of Correction: Training was completed and certificate was placed in the employee's file.

Standard #: 22VAC40-665-580-E-1
Description: Based on a review of records and interview, the vendor did not obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of the children in care within 90 days of employment.
Evidence: 1. The records of staff #1 (DOH 7/29/21), staff #2 (DOH 7/29/21), staff #4 (DOH 6/4/21), and staff #5 (DOH 5/15/21) did not contain documentation of certification in CPR. 2. Administration confirmed these staff haven't completed the certification.

Plan of Correction: First Aid and CPR will be completed for all employees on 12/21/2021.

Description: Based on a review of records and interview, the vendor did not ensure each staff member had a current certification in first aid within 90 days of employment.
