An unannounced SHSI inspection was conducted today from 11:15 am- 1:00 pm. Supervision and child ratios were in compliance. Vendor/ child information was reviewed. Emergency planning, supplies and drills were also inspected. Hand washing, diaper changing and self care routines were observed and discussed at length for the differing ages and stages of children in care. Areas of non- compliance were reviewed with plan of correction given at the time of the inspection. please contact kelly.savittiere@dss.virginia.govif there are any questions regarding this inspection.

Standard #: 22VAC4-665-340-H Description: Based upon observation and record review, crib in care did not meet CPSC requirements of 2012 or newer. Evidence: The manufacture date of crib being used by child in care was 7/2005. Plan of Correction: I will get.
Standard #: 22VAC40-665-160-B Description: Based on record review and interview, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: There was no documentation of 3 children arriving into care today. There was no Hard copy documentation of arrival and departure times as it occurs . Plan of Correction: Attendance was done during inspection. I will do from now on on form.
Standard #: 22VAC40-665-170-B Description: Based on information reviewed required child information was incomplete. Evidence: Children were missing emergency contact address and telephone numbers and parent work addresses. Plan of Correction: i will use new forms with address lines. I will get from parents
Standard #: 22VAC40-665-240-A Description: Based on observation, areas and equipment were not in safe and operable condition. Evidence: The door on the diaper changing table was hanging from its hinges and was directly in the child care area. Plan of Correction: I will fix.
Standard #: 22VAC40-665-370-E Description: Based upon observation, diaper changing surface was no longer considered to be "non-absorbent" as required. Evidence: Diaper changing pad had holes exposing padding allowing for bacteria to enter during cleaning. Plan of Correction: I will change.
Standard #: 22VAC40-665-370-F Description: Based upon observation, soiled diapers were not disposed of in a way to that neither caregiver's hand nor soiled diaper touches exterior surface. Evidence: Diaper trash can was located on patio. Vendor needed to open door with hands that had touched dirty diaper. Plan of Correction: I will buy a little one for inside.
Standard #: 22VAC40-665-390-A-1 Description: Based upon observation, required first aid kit was missing item. Evidence: There was no digital thermometer in first aid kit. Plan of Correction: I will buy new one.
