Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A subsidy health and safety (SHSI) inspection was initiated on 11/15/2021 and concluded on 11/17/2021. There were 24 children present with 4 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on Monday 12/27/2021. Please email me at morgan.bryson@doe.virginia.gov with any questions.

Standard #: 22VAC40-665-540-A
Description: Based on record review, the center did not ensure that all staff submit documentation of a negative tuberculosis screening or test at the time of employment and prior to coming into contact with children.

Evidence: Staff #2 (Date of Hire: 8/24/2021) did not have the results of a tuberculosis test on file at the time of inspection.

Plan of Correction: Got a copy of the TB test results and put in file.
