Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A subsidy inspection was initiated on 12/15/2021 and concluded on 12/15/2021. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. The center has not reopened since March 2020. The licensing inspector was unable to inspect all areas needed and will inspect when center reopens.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
