Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on December 15, 2021 and concluded on December 21, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 24 children present, ranging in ages from 4 months to five years of age, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on a review of staff records, the center failed to obtain a completed central registry finding within 30 days of employment.

Evidence:
1. Staff #3 did not have a completed central registry finding in the record. Date of hire for Staff #3 was 09/30/2021. There was a copy of the paperwork that was sent out on 11/22/2021 to obtain the central registry finding. That should have been sent out within the first week of hire.
2. Staff verified that there was not a completed central registry finding for Staff #3, and verified the date of employment.

Plan of Correction: Administration misunderstood the timeline to complete the central registry finding. In the future central registry requests will be sent out within the first week of hire. Once the central registry finding is obtained a copy will be sent to the Licensing Inspector.

Standard #: 8VAC20-780-130-A
Description: Based on review of children records, the center failed to obtain immunizations prior to a child attending.

Evidence:
1. Child #1's immunizations on file were dated 12/06/2021. Date of enrollment for Child #1 was 11/29/2021.
2. Staff verified the date on the immunizations and the date of enrollment for Child #1.

Plan of Correction: In the future administration will ensure that immunizations are in the child's record before the first day of attendance.
