Discussed with director:
Medication forms should list one medication per form. The name of the medication on the form should match the name of the medication present in the center. The dosage of the medication must be on the authorization form. Medications to be kept at the center for more than 10 days must have both a parental and a doctor authorization.

The current enrollment form does not have a place to document which staff member has verified the child's proof of age and identity.
