Technical Assistance:
New requirements became effective on October 13, 2021. The facility has not yet fully complied with the following requirements:
- Required new employee orientation topics and documentation
- Allergy management plans for those children with diagnosed food allergies
- record of children with diagnosed food allergies, sensitivities and dietary restrictions that is dated and indicating action(s) to be taken.
- - required procedures in the emergency preparedness plan and required documentation regarding evacuation, shelter-in-place and lockdown drills
- - written policies regarding the prevention of shaken baby syndrome and abusive head trauma including procedures to cope with crying babies or distraught children.
The facility is to review the new requirements and work with their assigned inspector to ensure future compliance

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on 01/19/2022 and concluded on 01/19/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 9 children present, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children's records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based upon review of documentation, the facility has not ensured that they obtain results of a follow-up tuberculosis screening at least every two years.
Evidence:
1. The most recent tuberculosis screening on record for staff 1 is dated 09/19/2018.
2. The most recent tuberculosis screening on record for staff 3 is dated 12/07/2019.
3. Staff 4 confirmed that more recent screening updates have not been obtained.

Plan of Correction: The facility responded with the following:
All staff will be required to submit results of a tuberculosis screening prior to two years since the most recent screening. A tracking system will be created to ensure that all staff records are up to date.

Standard #: 8VAC20-780-60-A
Description: Based upon review of documentation and staff interview, the facility has not ensured that each child's record includes the first date of attendance.
Evidence:
1. The record provided for child 1 did not have the first date of attendance indicated.
2. Staff 1 stated that child 1 enrolled on 09/27/2022 but did not start attending until December. Staff 1 acknowledged that the first date of attendance had not been indicated in child 1's record.

Plan of Correction: The facility responded with the following:
All children's records will be checked to ensure that the first (and last) dates of attendance are written in the record.
