22.1 Early Childhood Care and Education

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on January 14, 2022 and concluded on January 18, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 29 children present, ranging in ages from 2 years to 5 years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on a review of records and interview, the licensee did not ensure to obtain the results of an out-of-state search of the child abuse and neglect registry or equivalent registry from any state in which the staff previously resided within the preceding five years within the required timeframe.
Evidence: 1. The record of Staff #1, hired on 7/13/20, contained a sworn statement that indicated the staff member resided outside of Virginia within the preceding five years. The record did not contain documentation of results of a search of the out-of-state child abuse and neglect registry. 2. Administration acknowledged the results were not obtained.

Plan of Correction: Mailed new form on 1/18/22. VP/Financial Manager to make sure all out of state records are received before employment.

Standard #: 8VAC20-780-160-B
Description: Based on a review of three staff member's records and interview, the center failed to obtain an acceptable form of documentation of tuberculosis screening for two staff records.
Evidence: 1. The records of Staff #1 and Staff #3 contained documentation of a tuberculosis screening that was not signed by a physician, physician's designee, or an official of the local health department. 2. Administration acknowledged the screening was not signed.

Plan of Correction: Sending staff for new TB's that are signed. Director will make sure all TB's are signed before accepting.
