Description: Based on video observations, it was determined that areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
The Licensing Inspector observed video footage taken of the 2's classroom on 11/30/2021. The video footage shows that on two different occasions that day staff A swept the floor with a broom, then proceeded to wipe one of the children's tables with the same broom used to sweep the floor. This method does not keep the table clean.

Plan of Correction: Staff have been retrained on proper cleaning techniques.

Description: Based on staff interviews and a review of classroom video footage, it was determined that on one occasion, behavioral guidance was not constructive in nature, age and stage appropriate, and intended to redirect children to appropriate behavior and resolve conflicts.
Evidence:
On 11/30/2021 child X was injured while in care, and was later diagnosed by a physician as "having an episode of nursemaid's elbow" on his right arm. The Inspector observed on video footage that the child was seated at a table engaging in a table top activity when staff A and staff C asked all of the children to clean up and get their coats on to go outside. Instead of cleaning up, child X remained seated at the table. In the video footage, staff A is seen walking to the table and reaching for the child's left arm from across the table. Staff A then puts the child's left arm down, walks around the table and stands next to the child, then reaches for the child's right arm in an effort to get him to stand up and leave the table. Child X is then seen standing up, and while crying, walking with and reaching to be picked up by staff A. Staff A is seen holding child X by the arm while walking across the classroom holding a broom in her other hand. After eventually placing the broom down, staff A then picks up the child to comfort him and put his jacket on. At this point in the video, all children and staff are seen exiting the classroom for outdoor play time where there is no video.
After coming inside, child X is seen on video footage sitting at the table engaging in another table top activity. However, the child is seen clearly only using his left arm while keeping his right arm in his lap.
The classroom video footage, as well as information obtained during interviews with staff A, staff B, and staff C indicate that staff A engaged in inappropriate behavior guidance in her interactions with child X.

Plan of Correction: Staff have been retrained on the correct way to lift children and behavior management.

Standard #: 8VAC20-780-550-P
Description: Based on a review of one child's injuries, it was determined that center staff did not maintain a written record of serious and minor injuries in which entries were made the day of occurrence.
Evidence:
Child X obtained injuries on two different days.
1 - Incident 1 occurred on 11/18/2021. On this day, the child came home wearing a band-aid on his thumb. While home, a large cut was observed on the child's thumb. A written record of the injury was never completed.
2 - Incident 2 occurred on 11/30/2021. On this day, the child received an injury to his elbow. A written record of the injury was never completed.

Plan of Correction: Staff have been retrained on how to properly document incidents and accidents.
