Comments:
A self reported incident regarding behavior guidance was investigated on the above dates. After reviewing all the available fact the incident resulted in a violation notice getting issued to the center. The ongoing compliance with the standards is the responsibility of the licensee.

Standard #: 8VAC20-780-400-B
Description: Based on video evidence a staff failed to offer behavior guidance that was constructive and age appropriate.
Evidence: In the 54 second video, a recording from the centers in house video monitoring system, a child, age 2 years, in the early preschool room is observed sitting next to her cot, with her arms on the cot. The child then is sitting on her knees when staff 1 approaches her cot. Staff 1 is observed pushing the child onto the cot by placing her hand on the child?s back. Placing her on the cot, turning her over and pinning her to the cot with a hand on her head and another on her bottom. The staff repositions the child several time, moving her legs, flipping her over, moving the child's arms along the child's side , the child is observed trying to roll over and the staff continues to hold the child down on the cot. Staff 2 is standing next to the cot with a blanket observing the other staff.

Plan of Correction: The incident was brought to our attention on December 3rd, 2021. After being given the information, I watched the footage from our camera system. After watching the recording, I brought the staff member in to my office to show her the video, inform her that she was terminated effective immediately, and escorted her out of the school. Once the staff member had exited the building, I informed all of the families in the classroom that she would no longer be returning, contacted the family of the student involved to explain what happened, reported the incident to both CPS and licensing.
