Comments:
An unannounced complaint investigation was conducted on 12/15/2021 to investigate a complaint received by the department involving the area of records and reports. On the date of inspection, 131 children were in care, directly supervised by 26 staff.

On this date, observations were made, interviews were conducted with staff, documents were reviewed and staff/child records were reviewed.

The information gathered during the inspection did not support the allegations of the complaint so the complaint is determined to be not valid. Areas of non-compliance not related to the complaint are listed on the Violation Notice. Description: Based on record review and interview with the director, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain the last date of attendance.
2) The director had stated that Child #1 had been disenrolled sometime at the end of October.

Plan of Correction: Corrected during the inspection visit.
