Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on 12/16/21 and concluded on 01/18/22. There were 67 children present with 15 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on a review of five staff records and interview, the vendor did not ensure one staff was evaluated by a health professional and was issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the required time frame.

Evidence: 1) The record for Staff #1, hired on 08/30/21, did not contain documentation of a completed TB screening. 2) During interview, a member of management acknowledged the TB screening in the record for Staff #1 was incomplete, the form was not signed by a physician, physician's designee, or an official of the local health department. For staff hired prior to 10/13/21, documentation of the screening was to be submitted at the time of employment and prior to coming into contact with children.

Standard #: 22VAC40-665-580-B
Description: Based on a review of five staff records and interviews, the vendor did not ensure that within the first 90 days of employment, one staff who works directly with children, completed the Virginia Preservice Training for Child Care Staff.

Evidence: 1) The record for Staff #1, hired on 08/30/21, did not contain a certificate of completion of the VA Preservice Training for Child Care Staff. Staff hired prior to 10/13/21 were
required to complete the training within the first 90 days of employment. 2) During interview, a member of management confirmed Staff #1 has not completed the training.

Description: Based on a review of five staff records and interview, the vendor did not ensure one staff had documentation of current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 90 days of the date of employment.
