Technical Assistance:
Discussed hand washing procedures for children and staff after handling the hens residing on the outdoor play area.

Discussed a written plan for ensuring the outdoor play area is maintained in a clean and safe condition on a daily basis.

Effective 09/01/2021 all buildings built before 2015 are required to be equipped with at least one carbon monoxide detector.

Comments:
An unannounced monitoring inspection was conducted on 01/13/2022 from 11:50 am - 12:45 pm. At the time of entrance there were 11 preschool children in care with two staff. The children were observed during outdoor play. Two children's records and three staff records were reviewed vie electronic methods on 01/19/2022.

Violations were observed in the areas of building maintenance and the terms of the license. These areas of non-compliance are listed on the violation notice issued to the facility on 01/26/2022.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that outside areas and equipment of the center shall be maintained in a safe condition.

Evidence:
1. The following safety hazards were observed on the outdoor play area during the in-person inspection on 01/13/2022:
a. A garden hose was spread out, approximately 70 inches in length, on the ground where children were playing. This is a trip hazard as children were observed running and playing around this garden hose.
b. A stack of approximately 23 loose building blocks had been placed in a pile beside the building in the corner area adjacent to the stair way. A child fell and received a minor injury while attempting to climb on this pile of blocks;

Plan of Correction: 1 (a) Staff will ensure hose is rolled up after each use.
(b) Staff will ensure blocks are not stacked.

Description: Based on observation, the center failed to ensure that the findings of the most recent inspection of the facility shall be posted on the premises of the facility.

Evidence:
1. The findings of the 05/27/2021 monitoring inspection were not posted on the premises of the facility.
a. This is the most recent inspection of the facility.
2. The findings of inspections conducted on 08/26/2019 and09/28/2020 were posted.

Plan of Correction: 1 (a) Monitoring inspection has been posted.
