Orientation training documentation and Annual training (ongoing training) documentation needs to include name of staff, training
topic, training delivery method, individual providing training or entity, number of training hours or credit hours received and the date of training, Refer to standards 240 A-F and 245.

Discussed ? 22.1-289.058. Child day programs -Each building that was built before 2015 and that houses a child day program that is
licensed that serves preschool-age children shall be equipped with at least one carbon monoxide detector.

New TB test requirements - Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program .


A renewal inspection was initiated on 1/28/2022 and concluded on 1/28/2022. The director was contacted by telephone to initiate the inspection. There were 32 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-280-G
Description: Based on observation and staff interview, the licensee did not label the substitute container indicate contents.

Evidence:
There was a container with a liquid substance in the girls and boys bathroom . Staff confirmed it was soap. The container was not label with the content.

Plan of Correction: The director will ensure that containers will be labeled in the boys and girls restroom label (soap)

Standard #: 8VAC20-780-440-E
Description: Based on observation and staff interview, the licensee did not ensure there were at least 12 inches of space between occupied cots and rest mats.

Evidence: In the Two 1/2 year old classroom, there were 6 rest mats occupied by children that did not have at least 12 inches space between the rest mats. Confirmed by Program Director.

Plan of Correction: Director will ensure that staff will be retrained on state standard 22 VAC 40-185-440.
