Comments:

A monitoring inspection was conducted on 1/13/2022. There were 13 children present, infant through school age, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and staff FA/CPR and daily health observation training were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	8VAC20-780-270-A
Description:	Based upon observation and staff interview, the facility has not ensured that areas are maintained to be safe. Evidence: 1. The storage cabinet under the changing table in the infant area was not latched to be inaccessible to the children. There were numerous empty plastic grocery type bags in this storage area. 2. Staff 5 acknowledged that the storage area was not latched to be inaccessible to the children. 3. There were numerous cords to cell phones, a notebook, a computer and a diffuser lying across the floor under the table in the corner of the infant room. 4. Staff 5 acknowledged the accessible cords.

Description:	Based upon observation and staff interview, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children. Evidence: 1. There were hazardous substances, all with warning labels, that were accessible to the children. a. The low storage cabinet under the infant changing table was not latched to be inaccessible to the children. There were 2 unlabeled spray bottles of fluid, identified as cleaning agents by staff 5 who also acknowledged that the cabinet was not latched, in the storage cabinet. b. A container of pump hand sanitizer was unattended on the top of the shelf between the front room and the second room. c. A container of Aquafor was on a shelf in the second room from the front. d. A container of hand sanitizer was on the portable sink by the changing table. e. A container of Aquafor labeled for child 4 was on a shelf in the infant classroom. 2. Staff 1 acknowledged the presence of the above unlocked hazardous substances. 3. There were 2 school aged children in the back room. The door to the kitchen area was standing open. A container of Steramine sanitizing agent, labeled "warning, harmful if swallowed" was on the sink in the kitchen.
Plan of Correction:	The facility responded with the following: Spray bottles are now labeled. Cabinets are now locked. Hand sanitizer & lotion were removed and stored in locked cabinet. Kitchen door will remain closed & locked.

Description:	Based upon observation, the facility has not ensured that when hazardous substances are not kept in original containers, the substitute containers are clearly labeled to indicate their contents. Evidence: There were 2 spray bottles of fluid in the cabinet under the toddler changing table and two spray bottles of fluid in the cabinet under the infant changing table. None of the spray bottles were labeled as to the contents of the bottles.

Description:	Based upon review of the medication for children in care and staff interview, the facility has not ensured that there is parental and physician authorization for the administration of the medication. Evidence: 1.There was an albuterol inhaler, benadryl and an Auvi-Q epinephrine injector stored for administration to child 5. There was no written parental or physician authorization for the administration of the medications.

Description:	Based upon observation and staff interview, the facility has not ensured that prescription medication is maintained in the original, labeled container that was dispensed from a pharmacy. Evidence: There was an Auvi-Q epinephrine device that staff 1 identified as being for child 5. The Auvi-Q was not in a prescription box nor did it have a label from a pharmacy to include the child's name.

Description:	Based upon observation and staff interview, the facility has not ensured that medication is kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. There was a bottle of ibuprofen and a container of Benadryl on a shelf in the high cabinet in the open office area outside of the children's bathroom. The child proof latch was not latched on this cabinet. 2. Staff 3 acknowledged that the cabinet was not locked.

Standard #:	8VAC20-780-560-G
Description:	Based upon observation and staff interview, the facility has not ensured that when food is brought from home, the container is labeled with the child's name and the date. Evidence: There was a lunch bag on a shelf by the window in the front room. The name on the bag was not legible and the bag was not dated. Staff on duty stated that they were trying to determine to whom the bag belonged.

Standard #:	8VAC20-780-570-C
Description:	Based upon review of infant records, the facility has not ensured that the record for each child on formula includes the brand of formula and the infant's feeding schedule. Evidence: 1. The record for child 2, born in August 2021 and still drinking from a bottle, did not include the brand of formula or feeding schedule for the child. 2. Staff 1 verified that the required documentation of the brand of formula and sleep schedule was not available for child 2.

Standard #:	8VAC20-780-570-E
Description:	Based upon observation, the facility has not ensured that prepared infant formula is dated and labeled with the child's name. Evidence: 1. There were 2 prepared infant bottles on the shelf between the infant and toddler areas. Neither bottle had a child's name or the date on the bottle. 2. There were four feeder bottles containing what appeared to be applesauce on the same shelf . These bottles were dated 12/30. 3. There was an "Elmo" bottle laying on the floor in the fenced area of the infant room in which there were no children. The bottle was not labeled with a name or the date.
Plan of Correction:	Staff labeled all bottles. Feeder bottles were thrown out.
Standard #:	8VAC20-780-570-K
Description:	Based upon observation, the facility has not ensured that semisolid food is served to infants with a spoon unless there are written instructions from a physician or physician designee stating differently. Evidence: There were three infant feeder bottles containing what appeared to be applesauce on a shelf in the infant classroom. These bottles were labeled for three different infants (child 1, 2 and 3). Staff stated that the records for these three children did not contain instructions from a physician for the applesauce to be fed from a bottle and not by a spoon.
