Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program (if applicable).

A monitoring inspection was initiated on 2/7/2022 and concluded on 2/7/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 34 children present, ranging from infants to school age, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A-2
Description: Based on record review, the center did not ensure, three of ten staff records had documentation of a negative tuberculosis screening completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
1. The record of staff #3 (start date: 10/15/2021) had documentation of a negative tuberculosis screening dated 9/1/21.
2. The record of staff #5 (start date: 12/8/21) had documentation of a negative tuberculosis screening dated 12/23/2021.
3. The record of staff #10 (start date: 1/17/22) did not have documentation of a negative tuberculosis.

Plan of Correction: We will make sure they get it before they start. We will make sure staff #3 and staff #10 get a TB test this week.

Standard #: 8VAC20-780-40-M
Description: Based on record review, the center did not ensure to maintain a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8.
Evidence:
The center did not have a current list of all children's allergies, sensitivities and dietary restrictions.

Plan of Correction: Were going to make a list with all kids in the center and also ask parents when they start for an allergy care plan from the doctor.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not ensure one of three children's records contained documentation of a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
The record of child #3 (start date: 1/31/22) did not have documentation of a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. The record of child #2 had documentation of a diagnosed food allergy.

Plan of Correction: Were going to make a list with all kids in the center and also ask parents when they start for an allergy care plan from the doctor.
