22.1 Early Childhood Care and Education

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was conducted on February 8, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were twenty-two children present, ranging in ages from infants to age four, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review, facility did not ensure a required out of state background check is completed prior to staff beginning employment, when an individual has lived outside the state of Virginia within the past 5 years.

Evidence: The record for staff #1 (start date September 30, 2021) had documentation of living outside the state of Virginia within the past 5 years and there was not a completed sex offender search for that state.

Plan of Correction: A sex offender search will be completed for that state and results placed in the staff record. In the future, this will be completed prior to beginning employment.

Standard #: 8VAC20-780-130-A
Description: Based on review, facility did not ensure 1 of 3 children records has an immunization record prior to child beginning attendance.

Evidence: The record for child #1 (start date September 30, 2021) did not have an immunization record.

Plan of Correction: Parents have been notified to provide documentation.

Standard #: 8VAC20-780-140-A
Description: Based on review, facility did not ensure 2 of 3 children records has a physical examination prior to beginning attendance or within 30 days of beginning attendance.

Evidence: The records for child #1 (start date September 30, 2021) and child #2 (start date November 8, 2021) did not have a physical examination.

Plan of Correction: Parents have been notified to provide documentation.

Standard #: 8VAC20-780-160-A
Description: Based on review, facility did not ensure 2 of 3 staff have results of a negative tuberculosis screening.

Evidence: The records for staff #1 (start date September 30, 2021) and staff #2 (start date January 6, 2022) did not have documentation of a negative tuberculosis screening.

Plan of Correction: Staff will have TB test/screenings completed.

Standard #: 8VAC20-780-260-A
Description: Based on review, facility did ensue there is an annual fire inspection report.

Evidence: The most recent fire inspection report was completed June 24, 2020 which is more than one year old.

Plan of Correction: Director has requested a fire inspection be completed.

Standard #: 8VAC20-780-280-B
Description: Based on observation, facility did not ensure potentially hazardous chemicals are kept locked.

Evidence: Cleanser and bleach/water solution were stored on top of the paper towel dispenser in the school age classroom.

Plan of Correction: Staff will be reminded to keep chemicals in the wooden lock box on the wall in the classroom.
