Comments:
An unannounced renewal inspection was conducted on 2/03/2022 and 2/07/2022. At the time of entrance, 53 children were in care with 9 staff members present. Children were observed playing in centers, story time, painting and outside time. The site was clean, organized and contained a supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child B was listed on the allergy list to have a food allergy, but the center did not have a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: Parents were given Food Allergy and Anaphylaxis Emergency Care Plan for the doctor to complete with instructions. It was returned 2/8/22 with the instructions to "call parents if ingested."

Standard #: 8VAC20-780-270-A
Description: Based on observation, the areas and equipment of the center inside were not maintained in a safe and operable condition.
Evidence:
1. In the school age classroom, there was a small hole in the blue sofa.
2. In the 2nd school age classroom, there were 8 tiles with rust on it.
3. In the 3rd school age classroom, the sliding closet door was off track and needed to be fixed.

Plan of Correction: 1. Sofa will be replaced, ordered within a week. 2/16/22
2. Church building trustees were contacted to put in a work order for the tiles to be cleaned or replaced. 2/09/22
3. Work order was put in for contractor to fix door as soon as possible. 2/09/22
