Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on and concluded on November 23, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 18 children present, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 63.2(17)-1720.1-B-4
Description: Based on review of four staff records and interview, the center did not obtain a sex offender registry check from all states in which one staff member had resided within the last 5 years prior to employment.

Evidence:
The record for Staff #2 (Date of Hire: 04/26/2021) indicated the staff member had resided in another state within the last five years. The record did not contain a sex offender search from the state indicated in the record. A member of management confirmed this during the inspection.

Plan of Correction: We will request the sex offender search immediately.
