63.2 Child Abuse & Neglect

Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:15am-10:30am. There were a total of 15 children with four staff members present. Children were observed during snack time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-770-60-C-2
Description: Based on record review, the licensed or registered child welfare agency or family day system does not have a central registry finding within 30 days of employment or volunteer service.
Evidence:
1. Volunteer #3 (Start date- 9/15/2020) did not have documentation of a Central Registry finding in the record.

Plan of Correction: Management will contact VDSS to locate a copy of the Central registry.

Standard #: 8VAC20-780-40-K
Description: Based on record review, the center did not develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence:
1. The center did not have documentation of written procedures for shaken baby syndrome or abusive head trauma.

Plan of Correction: The center will develop a written plan for head trauma.

Standard #: 8VAC20-780-80-A
Description: Based on record review and observation, for each group of children, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. The written record of attendance documented the arrival of the children, but did not document the departure of the children on the attendance record.

Plan of Correction: Management will add an additional column to the attendance record to indicate departure of the children.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides was not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A bottle of bleach sanitizing wipes was not locked up and located on a low shelf accessible to children.

Plan of Correction: Cleaning products will be locked up.

Description: Based on record review, the center did not develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.
Evidence:
1. The center did not have documentation of a written policy and procedure that described how the center would ensure that each group of children receives care by consistent staff or team members.

Plan of Correction: The center will develop a plan of continuity of care plan.

Standard #: 8VAC20-780-550-G
Description: Based on record review, required documentation was not maintained of emergency evacuation, shelter-in-place, and lockdown drills.
Evidence:
1. The Emergency evacuation and shelter in place drill logs were missing the following information: method used for notification, number of staff, special conditions, time it took to complete drill, problems, if any, and weather conditions for emergency drills only.

Plan of Correction: The Evacuation Log will be updated to add required documentation.
