22.1 Early Childhood Care and Education

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was initiated on 2/10/2022 and concluded on 2/15/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 22 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review, the center did not ensure that all staff undergo a repeat Virginia Central Registry of Child Abuse and Neglect search every five years.

Evidence: Staff #2's (Date of Hire: 4/1/2016) most recent Virginia Central Registry of Child Abuse and Neglect search results on file were dated 11/7/2016.

Plan of Correction: All staff files will be reviewed and calendar invites sent to staff by March 31, 2022. Staff #2 has completed form and it is being mailed today.

Standard #: 8VAC20-770-60-B
Description: Based on record review, the center did not ensure that all staff complete a Sworn Disclosure Statement prior to employment.

Evidence: Staff #1 (Date of Hire: 1/31/2022) had a Sworn Disclosure Statement on file dated 2/3/2022.

Plan of Correction: New hires will sign sworn statement prior to first day.

Standard #: 8VAC20-780-130-A
Description: Based on record review, the center did not obtain immunization records for each child before the first day of attendance.

Evidence: Child #3 (First Day of Attendance: 8/23/2021) does not have immunizations on file.

Plan of Correction: Create checklist for new admissions by 2/28/22 to add to daycare packet. Will have immunizations by 2/18/22.

Standard #: 8VAC20-780-160-A-2
Description: Based on record review, the center did not obtain documentation of a negative tuberculosis screening completed within the 30 calendar days prior to the date of employment for all staff.

Evidence: Staff #1 (Date of Hire: 1/31/2022) had a tuberculosis screening on file completed 8/20/2021 and 2/13/2022. Staff #3 (Date of Hire: 10/4/2021) had a tuberculosis test on file completed 9/28/2020.

Plan of Correction: TB test dated no more than 30 days prior to hire date will be obtained for all staff. Staff #1 on file 2/15/22. Staff #3 in progress will have by 2/21/22.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center did not ensure that each enrolled child has all required information on file.

Evidence: Child #4's record is missing phone numbers for 2 of 2 required emergency contacts.

Plan of Correction: Create a checklist for new admissions by 2/28/22 to add to daycare papers. Child #4's is corrected.
