Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews. A monitoring inspection was initiated on February 7, 2022 and concluded on February 8, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 8 children present, ranging in ages from 1 year to 4 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #:	22.1-289.058
Description:	Based on interview, the center did not ensure the building was equipped with a carbon monoxide detector. Evidence: Administration stated the center did not have a carbon monoxide detector.
Plan of Correction:	Carbon monoxide detector will be purchased 2/9/22 and placed in center by the end of the day 2/10/22.
Standard #:	8VAC20-770-60-C-2
Description:	Based on a review of staff records and interview, the center did not ensure that each staff record reviewed contained a central registry finding within 30 days of employment. Evidence: 1. The record of staff #1, hired on 10/4/21, contained documentation of a central registry finding dated 12/15/21. 2. The record of staff #2, hired on 12/2/21, did not contain documentation of a central registry finding. 3. Administration acknowledged one was late and the other she did not have.
Plan of Correction:	The employee has provided a signed and notary stamped cps form that will be mailed out 2/8/22.
Standard #:	8VAC20-780-340-D
Description:	Based on a review of staff records and interview, the center did not ensure in each grouping of children at least one staff member who meets the qualification of a program leader or program director is regularly present. Evidence: 1. During interview, it was stated that staff #1 was the lead teacher in the after school classroom. The record of staff #1 did not contain documentation of lead teacher qualifications. 2. During interview, it was stated that staff #2 was the lead teacher in the infant classroom. The record of staff #2 did not contain documentation of lead teacher qualifications. 3. Administration acknowledged that neither staff in the classroom met the lead teacher qualifications.
Plan of Correction:	Center will have lead teacher in place 3/4/22 with 24 hours of training from Better Kid Care completed on 2/15/22. 2nd teacher will be an aide until employed 6 months at the establishment and will take 5 courses to complete 24 hours of training from Better Kid Care.
