The program has been given a list of items needed so that the application can be deemed complete.

Comments:
This inspection was conducted by licensing staff and staff used two components to conduct the inspection.

A renewal inspection was initiated on February 17, 2022 and concluded on February 22, 2022. The designated liaison was contacted by email in order to request documents and the inspection was in person during operating hours. There were 13 school aged children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The staff were also observed releasing children to parents in the course of the inspection. A total of 4 child records and both staff records were reviewed. The staff
were able to answer questions related to the program and the children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please call with questions 703-479-4679

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on record review and interviews, it was determined the program is not ensuring that its policy regarding medication are maintained in compliance with the standards and the center's own policies and procedures.
Evidence:

1.The Center requires that parents provide written authorization to the program to administer when their children may need medication and it is stored at the center. Child #1 had 3 types of medication at the center but one medication didn't have the parent's written authorization to administer it. Child #3 had written authorization for 2 medications but did not have the medication and only had the medication on hand for child #3's sibling.

Plan of Correction: We will request the medication and the authorization form from each parent.

Standard #: 8VAC20-780-510-G
Description: Based on record review and interview, it was determined the center staff didn't ensure medication was labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given for all the children with medication issues.
Evidence:
1. Child #1 did not have the over the counter medication labeled with the child's name, dosage amounts and when it should be given.
2. Child #2 had prescription medication without the prescription label to identify child's name and dosage.

Plan of Correction: Will request the information from each family.
