Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:45am-11:45am. There were a total of 83 children with 16 staff members present. Children were observed engaged in group activities, hand washing, small groups, and singing . A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #5's record (Start date-02/04/22) did not contain documentation of proof of identity.

Plan of Correction: The center will view the birth certificate and document the child's record.

Standard #: 8VAC20-780-240-A
Description: Based on record review, staff did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence:
1. Staff # 2's record (Date of hire-11/2/21) did not contain documentation of a completed training of the Virginia Department of Education sponsored orientation course.

Plan of Correction: The staff member will complete the required training and the center will document the record.

Standard #: 8VAC20-780-350-F
Description: Based on record review and interview, the center did not develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.
Evidence:
1. The center did not have documentation of a continuity of staff plan for each group of children.
2. Staff #5 confirmed that the continuity of care plan was unavailable to review.

Plan of Correction: A continuity of care plan will be written and developed.

Standard #: 8VAC20-780-550-G
Description: Based on record review, emergency evacuation logs did not contain required information.
Evidence:
1. The Evacuation logs that were provided for review were missing the following information: method used for notification, any special conditions simulated, time it took to complete drill, problems encountered, if any,, and weather conditions for evacuations only.

Plan of Correction: The center will use the Department's model forms to be in compliance.
