Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:30am-11:00am. There were a total of five children with four staff members present. Children were observed engaged in group activities, hand washing, and library time. A selection of staff and children records and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-780-40-K
Description: Based on record review and interview, the center did not develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence:
1. The center did not have documentation of written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
2. Staff #2 confirmed that written procedures for prevention of shaken baby syndrome or head trauma had not been developed.

Plan of Correction: The center will develop written procedures for shaken baby syndrome or head trauma.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A bottle of bleach and water was observed on a high shelf not accessible to children, but not kept in a locked place using a safe locking method that prevents access by children.

Plan of Correction: The bottle of bleach and water and soapy water solution was locked up during the time of the inspection.
Moving forward, cleaning and sanitizing substances will be locked up.
