This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in-person component. A monitoring inspection was initiated on 02.22.2022 and concluded on 02.23.2022. The director was contacted by telephone to initiate the inspection. The Inspector reviewed 5 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-340-A Description: Based on observation and interview, the center did not ensure the care, protection and guidance when supervising children.
An baby was observed in the infant room sleeping in a boppy pillow. There was a blanket covering the baby's entire face. When the inspector expressed concern as to why the blanket was covering the baby's face, staff stated that they knew the blanket should not be over the baby's face, however that was "the only way the child would fall asleep."Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director will talk with staff in the infant room about safe sleep.
Standard #: 8VAC20-780-440-E Description: Based on observation the center did not ensure there was at least 12 inches of space between occupied cots.
There were 20 children observed napping on cots and there was less than 12 inches of space between each. Some cots were were touching each other, leaving no space between them.Plan of Correction: The following corrective action was provided by the director during the exit interview:
The school age room will be used as additional space to ensure children can be spread out during nap. Staff will ensure there is at least 12 inches of space between each cot.
