The new Standards for Licensed Child Day Centers is available online at the Virginia Department of Education website.

Comments:
An unannounced renewal inspection was completed on March 3, 2022. There were 30 children present with 7 staff supervising. The children were observed working in small groups, individual mat work, having morning snack and playing outside. The physical plant, playground, medications, medication authorizations, evacuation logs,
snack menu's, lunch boxes, emergency supplies and child and staff records were reviewed. Areas of non-compliance were cited on the Violation Notice. If you have any questions, please contact the licensing inspector at tanya.johnson@doe.virginia.gov or call 571.423.6392.

Standard #: 8VAC20-780-160-C
Description: Based on a review of staff records, the center failed to ensure that at least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening .

Evidence:
Staff #2 and #3's records do not contain documentation of a current tuberculosis screening.

Plan of Correction: We will advise staff to have follow up tuberculosis test or screening.

Description: Based on a review of records, the center is non-complaint with it's policy to administer medication.

Evidence:
The center did not have a MAT person on staff between 9:45 am and 1:00pm.

Plan of Correction: We will have our MAT trained staff complete their re-certification.

Standard #: 8VAC20-780-240-D
Description: Based on a review of staff records, the center failed to ensure that within 90 days of employment staff should receive training on recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by 63.2-1509 of the Code of Virginia.

Evidence:
Staff #2's record did not contain documentation of completion of the Recognizing Child Abuse and Neglect training.

Plan of Correction: We will ensure that all staff have the Recognizing Child Abuse and Neglect Training.

Description: Based on a review of records, the center failed to obtain an annual fire insepction.

Evidence:
The Fire Inspection Permit expired on 09/30/2019.

Plan of Correction: The new fire inspection is set for March 8, 2022.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances are kept in a locked area not accessible to children.

Evidence:
In the Toddler Room, the cabinet with cleaning material was left unlocked.
In the Toddler Room and the Primary Room, there were hand sanitizers labeled "keep out of reach of children.

Plan of Correction: The cabinet was locked during the inspection and hand sanitizers were put in locked area.

Description: Based on observation, the center failed to ensure that each bathroom is equipped with soap, paper towels and toilet paper.

Evidence:
The bathroom in the Primary classroom did not contain soap.

Plan of Correction: Soap was placed in the bathroom during inspection.

Description: Based on review of evacuation drill logs, the center failed to ensure that practice evacuation drills are conducted monthly.

Evidence:
There is no documentation for a practice evacuation drill for March 2021.

Plan of Correction: We will ensure that drills are conducted monthly.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence:
In the Primary classroom there were 9 lunch boxes with no name, 1 lunch box with no date and 2 lunch boxes without a name or a date.

Plan of Correction: We will remind the parents to put the child's name on each lunch box, if the parent forgets, staff will ensure that all lunch boxes are labeled with name and date.

Description: Based on observation, the center failed to ensure that bottles are labeled with the required information.

Evidence:
A bottle in the Toddler classroom was not labeled with the child's name or the date.

Plan of Correction: We will remind parent's to label bottles daily,
