A monitoring inspection was initiated on 03/03/2022 and concluded on 03/03/2022. The inspector was on site at the center from 10:15am to 1:15pm. There were 17 children present, ranging in ages from 2 years to 4 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-K
Description: Based on record review, the center did not ensure to develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Standard #: 8VAC20-780-330-B
Description: Based on observation, the center did not ensure where playground equipment is provided, resilient surfacing complied with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 and was under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.
Evidence:
1. The center's wooden stationary equipment was 3 feet and 3 inches from the dome climber. There should be at least six feet of space on all sides of stationary equipment.
2. The fall zone for the tire swing and rope swing was 9 feet 4 inches. The required fall zone for the swings are twice the height of the swing beam in the front and back of the swing which is 14 feet and 4 inches.
3. The merry-go-round and alligator see saw did not have any resilient surfacing underneath and both pieces of equipment required at least 6 inches of resilient surfacing.

Description: Based on observation, the center did not ensure children under 10 years of age were always within actual sigh and sound supervision of staff.
Evidence:
During the inspection, the children in the fours classroom left the room on several occasions and went to another room or the hallway. The children were not within sight supervision of staff when leaving the room.
