Technical Assistance:
Discussion of the new regulations that went into effect 10/13/2021 and a review of medication on site.

Comments:
This renewal inspection was initiated and concluded on 03/03/2022, between the hours of 9:30 am through 11:30 am. The Director was originally contacted by telephone in order to obtain preliminary documentation. There were 24 children present, ranging from infant to preschool-age, with 5 staff supervising in 3 separate classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, and 7 staff records were reviewed.

Information gathered during the inspection determined that there were three (3) non-compliances with the applicable standards, code, and/or law. These violations were documented on the Violation Notice issued to the program.

Violations:
Standard #: 22.1-289.058
Description: Based on observation and interview, the Child Day Center was not equipped with at least one carbon monoxide detector on site.

Evidence -
1. A carbon monoxide detector was not observed on site. This was confirmed by the Center Director.
2. The Center Director stated (during an interview conducted on the date of inspection - 03/03/2022), that the building was built prior to 2015.
3. The center serves children who are preschool-age.

Plan of Correction: A carbon monoxide detector will be purchased, and then installed in the center.

Standard #: 8VAC20-770-40-D-3-d
Description: Based on review, 1 of 7 staff files indicated that it has been more than (5) five years since the date of the last search of the central registry (CPS check).

Evidence -
1. On the date of inspection (03/03/2022), the most current CPS check on file for Staff #1 was dated - 08/15/2016.
2. This documentation would have expired after 08/15/2021.

Plan of Correction: This documentation has been sent to OBI. The center will follow-up with OBI to find out about its status. When the findings are received, it will be shared with the Licensing Inspector.

Standard #: 8VAC20-780-160-C
Description: Based on review, 3 of 7 staff files indicated that it has been more than two years from the date of the last tuberculosis (TB) screening or testing (or more frequently as if recommended by a licensed physician or the local health department).

Evidence -
1. On the date of inspection (03/09/2022), the following staff had expired TB documentation on file at the center: Staff #1 (dated - 09/26/2017); Staff #4 (dated - 11/25/2019); and Staff #6 (dated - 09/07/2018).
2. This same violation was cited during an inspection conducted on 09/30/2021. The TB documentation for both Staff #1 and Staff #6 were cited at that time.

Plan of Correction: Updated TB documentation will be obtained for all three staff.
