Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on March 4, 2022 and concluded on March 7, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 24 children present, ranging in ages from five months to five years of age, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A-1
Description: Based on review of staff records, the center failed to ensure that staff had submitted a tuberculosis screening/test before coming into contact with children.

Evidence:
1. Staff #3's tuberculosis screening was dated 02/07/2022. Date of employment for Staff #3 was 09/23/2021.
2. Staff verified the date of employment for Staff #3 and the date on the tuberculosis documentation.

Plan of Correction: In the future all staff will obtain tuberculosis screening before working with a group of children.

Standard #: 8VAC20-780-260-A
Description: Based on review of the fire inspections, the center failed to obtain an annual fire inspection report.

Evidence:
1. The fire inspection on file was dated 11/24/2020. A new fire inspection was required by 11/24/2021.
2. Staff verified the date of the fire inspection on file, and confirmed a new one had not been completed.

Plan of Correction: Administration will contact the fire marshal and request an inspection. Once the fire inspection is obtained the provider will send it to the Licensing Inspector.

Standard #: 8VAC20-780-260-B
Description: Based on review of the health inspection, the center failed to ensure an annual health inspection was completed.

Evidence:
1. The health inspection on file was dated 11/2/2020. A new health inspection was due by 11/2/2021.
2. Staff verified the date on the health inspection and confirmed that a new health inspection had not been obtained.

Plan of Correction: Administration will contact the local health department and request an inspection be completed. Once completed the provider will send the inspection report to the Licensing Inspector.
