One slide on the playground exceeds the allowable height for preschool children. This slide is used for school age only.

Comments:
An unannounced Renewal inspection was conducted today. A total of 126 children with 21 staff were present. Observations of Classrooms 5,7,8 and 13 were completed. Both playgrounds were inspected. A review of some medications and topical ointments within the classroom was made. Staff interviews conducted, 4 staff records and 4 children's records were reviewed. Some violations were found today. Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation of the center, areas of the building were not maintained in a clean condition.
Evidence:
1. The wall near the trash can was observed dirty in Class 5.
2. The refrigerator in Class 7 was dirty with spilled food.
3. The diaper surface area in Class 7 was dirty.

Plan of Correction: We will review best practices around cleaning with Classrooms 5 and 7 immediately. All classrooms will participate in a workshopping session where their daily cleaning checklist will be reviewed, revised, and discussed to ensure everyone understands their role in keeping our environments clean and sanitary over the coming month.

Standard #: 8VAC20-780-330-B
Description: Based on observations of the playground, the resilient surface does not comply with the minimum safety standards.
Evidence:
The resilient surfacing material located under the playground equipment and the fall zone areas do not comply with the minimum 6 inches of resilient surfacing material.

Plan of Correction: Playground mulch will be redistributed to ensure a minimum of 6 inches in fall zones by Monday, March 14th. By March 14th, additional playground mulch will be scheduled for delivery if needed to maintain adequate depth.

Standard #: 8VAC20-780-350-B
Description: Based on observation, staff to child ratios were not maintained.
Evidence:
1. The Licensing Inspector observed a staff outside classroom 13 labeling lunchboxes
leaving 15 children remaining in the class with only 1 staff.
2. This classroom has mixed ages but it not a balanced mixed aged group. The youngest
child in the class is 3 years old. Therefore, a 1 to 10 ratio is required.

Plan of Correction: Staff will be reminded that they must maintain sight and sound awareness of children at all times in order to count in staff:child ratios on Friday, March 11th.

Description: Based on review of medications onsite, one medication was not properly labeled.
Evidence:
An antihistamine located in Class 5 was not labeled with the child's name.

Plan of Correction: An audit of all medications will be completed by Thursday, 3/17 to ensure that all medications are properly labeled.

Description: Based on observation of morning snack, food offered was considered a choking hazard.
Evidence:
1. Class 5 provides care for children under the age of 2.
2. Whole bananas were offered to the children for morning snack.
3. A 19 month old was observed eating a whole banana presenting a choking hazard.

Plan of Correction: Staff will be re-trained school policies requiring that food for children under age 3 be cut into pieces no more than ? inch square. This will be addressed immediately.

Standard #: 8VAC20-780-560-G
Description: Based on observation of food brought from home, one item was not properly labeled.
Evidence:
A sealed milk cup brought from home was found in the refrigerator in Class 5. This cup was not labeled with today's date.

Plan of Correction: All staff will be immediately reminded of the need to ensure that all food and beverages sent from hom e have been labeled with the child?s name and date. Parents reminders to label at home will be sent as well.

Standard #: 8VAC20-780-560-L
Description: Based on observation, staff were not sitting while children were eating.
Evidence:
In Class 5, the children were having morning snack. The staff were not sitting with the children while they ate.

Plan of Correction: All staff will be immediately reminded of the requirement to sit with children during mealtimes and group snacks.
