Technical Assistance:
Discussion of the new regulations that went into effect on October 13, 2021.

The Staff #1 and the Licensed Program Manager sent the Licensing Inspector preliminary information prior to the date of inspection for their review.

Comments:
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

This inspection was initiated and concluded on 03/10/2022. The Director was contacted and a virtual inspection was conducted. There were 32 school-age children present, with 3 staff supervising (the Site Supervisor, the Site Leader, and a Recreation Specialist). The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, and 2 staff records were reviewed.

Information gathered during the inspection determined that there were (2) two non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the program.

Standard #: 8VAC20-780-60-A
Description: Based on review, 2 of 5 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (03/11/2022), the parent's work information (place of employment and work phone number) for Child #2 was not available for review.
2. On the date of inspection, the start date for Child #5 was not available for review.
3. Both attempted reviews were confirmed by the Recreation Specialist on site.

Plan of Correction: All missing information will be obtained and placed on file.

Standard #: 8VAC20-780-420-E-3
Description: Based on review and interview, 1 of 5 children files indicated that the annual parent confirmation that the required information in their child's record was up to date was not documented.

Evidence -
1. On the date of inspection (03/10/2022), the date of the last review of the child record for Child #5 could not be determined.
2. The Recreation Specialist stated (during an interview conducted on the date of inspection) that the space for the date (as indicated on the short form) was left blank.

Plan of Correction: This parent will be contacted for the review and signature will be obtained.
