Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

An in-person monitoring inspection was conducted on March 3, 2022. There were 18 children present, ranging in ages from 18 months to 5 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed participating in circle time and heading outside to play. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: Child 1 does not have documentation that each child has received immunizations.

Plan of Correction: We will reach out to the parent.
