Comments:
The licensing inspector conducted an unannounced monitoring inspection on 02/23/2022 in response to a self reported incident allegations in the area of administration and programs. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-E
Description: Based on a record review and interviews, the center did not ensure the center's activities were maintained in compliance with the standards and the center's own policies.

1. On 02/22/2022, around or about 7:45am, Child A was transported by Staff 1 from the center to a public school she does not attend. The director confirmed Staff #1 did not follow the center's policy. It was reported Staff #1 did not perform face-to-name checks when the children were dropped off at public school, consequently, allowing child A to exit the bus at the wrong destination.

2. The center's standard operating procedures and health and safety guidelines states the "Child Safety Record (CSR), "is one of the many tools used to assist in properly supervising children." It further states it is designed to assist in keeping an accurate of all children physically in attendance."

3. The CSR instructions require staff to record a "face-to-name" check for each child by completing a CSR sheet when leaving the center and entering the bus. It further requires staff to record a "face-to-name" check for each child when exiting the bus to the destination.

Plan of Correction: The following corrective action was provided by the program director during the exit interview:
Staff received a "final coaching" and was retrained in the use of the CSR sheets. For the past several weeks the director checks the bus driver's CSRs before getting on the bus and upon return.

Standard #: 8VAC20-780-340-A
Description: Based on interviews and a record review it was determined the center did not ensure the care, protection and guidance while supervising children.

1. On 02/22/2022, a 5 year-old child was transported by the daycare center and left at a public school she does not attend.

2. Around or about 7:45am, the driver, Staff #1 left the center to transport a group of ten children to the public schools they attend during school hours. When the center's bus arrived at School C, five children exited the bus, to include 5 year-old Child A, who does not attend that school, rather she attends School D.


3. Staff #1 and the center were unaware Child A had been dropped off at the wrong destination, until the center received a call from Child A's mother. School C called the child's mother around or about 8:15am notifying her that her child had been left by the daycare center's driver at their school, instead of School D. School D sent a bus to pick-up Child A and transport her to the correct destination.
