Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:40am-11:45am. There were a total of 36 children with 12 staff members present. Children were observed during handwashing, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-780-160-A-1
Description: Based on record review, each staff member did not submit documentation of a negative tuberculosis(TB) screening at the time of employment.
Evidence:
1. Staff #1(Hired 10/26/21) did not obtain a negative tuberculosis screening until 11/2/2022.
2. Staff # 4 (Hired 10/14/2021) did not obtain a negative TB screening until 10/26/21.

Plan of Correction: Newly hired staff will submit documentation of a negative TB screening at the time of employment and prior to coming in contact with children.

Description: Based on record review, staff did not complete a Tuberculosis screening within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #5's record (Start date-02/09/2022) did not have documentation of a completed tuberculosis screening within 30 days of employment. The tuberculosis screening was dated 12/02/2021.

Plan of Correction: New employees will submit Tuberculosis screening documentation within 30 days of the date of employment.

Description: Based on record review, the center did not have documentation of viewing proof of the child's identity and age.
Evidence:
1. Child #3's record (Start date-02/28/2022) did not contain documentation of viewing proof of the child's identity and age.

Plan of Correction: The center will obtain the birth certificate from the parents and document the record.

Description: Based on record review, staff did not complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care within 30 days of the first day of employment.
Evidence:
1. Staff # 1 (Start date-10/26/21), Staff #3 (Start date-11/4/21), and Staff #4 (Start date-10/14/21) did not have documentation of a completed orientation training in first aid and CPR training within 30 days of the first day of employment.

Plan of Correction: Moving forward, staff will complete orientation training in first aid and cpr training within 30 days of employment.

Description: Based on observation, areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. In the infant and toddler classrooms, there was eye level and quarter sized peeling paint along the walls where the children are cared for.

Plan of Correction: A work order for the peeling paint for both classrooms have been put into maintenance.

Evidence:
1. Cots in the Toddlers, Twos, Early Preschool, Preschool, and Pre-K classrooms were not labeled to identify use for specific children.

Plan of Correction: Staff will label cots for each child in each classroom.

Description: Based on record review, the emergency plan was not reviewed and updated annually.
Evidence:
1. The Emergency Preparedness Plan was not updated annually. The Emergency Preparedness Plan had not been updated since December 2020.

Plan of Correction: The Emergency Plan will be reviewed and updated immediately.
