Comments:

A renewal inspection was initiated on 03/04/2022 and concluded on 03/09/2022. The facility submitted documentation to the inspector on 03/04/2022 and the inspector conducted an unannounced inspection on-site on 03/09/2022 from approximately 2:15pm to 5:20pm. On 03/09/2022, there were 86 children present with 15 staff directly supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and four staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the `plan of correction' and `date to be corrected' for each violation cited on the Violation Notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #:	8VAC20-780-130-A
Description:	Based on a review of six children's records, the center did not obtain documentation that one child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: 1) The immunizations in the record for Child #1, enrolled on 09/07/2021, were dated 10/11/2021.
Plan of Correction:	Per the Center: "On the date of the licensing inspection, the paperwork was correct in the file. The violation occurred because the paperwork was obtained after the child?s first day of enrollment. The child was adequately immunized at the time of enrollment. The violation came into play as the paperwork was not on file at the school at the time of enrollment. There is nothing further the center can do at this time as the file has been previously corrected. All administration has been retrained and we will use our enrollment checklist to monitor continued compliance with this standard. The school?s Administrative Coordinator will be responsible for ongoing compliance with this standard."

Description:	Based on a review of six children's records, the center did not have a physical examination by or under the direction of a physician for one child before the child's attendance or within 30 days after the first day of attendance. Evidence: 1) The physical examination in the record for Child #1, enrolled on 09/07/2021, was dated 10/11/2021.
Plan of Correction:	Per the Center: "On the date of the licensing inspection, the paperwork was correct in the file. The violation occurred because the paperwork was not obtained within the child?s first 30 days of enrollment. The child?s physical was completed prior to enrollment and within the required timeframe. The violation came into play as the paperwork was not on file at the school within 30 days of enrollment. There is nothing further the center can do at this time as the file has been previously corrected. All administration has been retrained and we will use our enrollment checklist to monitor continued compliance with this standard. The school?s Administrative Coordinator will be responsible for ongoing compliance with this standard."
