Technical Assistance:
A discussion was held on taking the children's bottles/cups apart before washing or send all bottles/cups home each day. Cot sheets need to laundered weekly, infant crib sheets must be laundered daily. Children's personal blankets can be sent home once a week to launder. Staff needs to ensure they are labeling and dating any container holding food/drink from home upon arrival.

Comments:
An inspection was conducted on Wednesday, March 23, 2022 investigating a complaint received in the licensing office concerning the cleanliness of the cups/bottles the children are using. Classroom observations and interviews with staff and management was conducted. The complaint was deemed valid. Violations related to this complaint along with other violations can be reviewed on the violation notice.

Description: Based on observation, each restroom was not provided with disposable hand towels.
Evidence
In the bathroom by the Infant room, there are not disposable towels or anything to dry hands on.

Plan of Correction: Each morning we will ensure each restroom has paper towels.

Description: Based on observation and conversation with staff, the center did not have a daily record that can be easily accessed by both the parent and the staff working with the child.
Evidence
For child #2 who was sitting in their chair drinking a bottle, did not have a daily record available for review. The time child arrived or how much they were drinking could not be determined.

Plan of Correction: The child just arrived and the sheet had not been made yet, In the future daily sheets will be made as soon as the child arrives and all important information will be documented.

Description: Based on conversation with management, infant sheets are not being washed daily.
Evidence
Management stated infant crib sheets are being washed twice a week. Infant sheets must be washed daily.

Plan of Correction: We will begin washing the sheets daily.

Description: Based on observation when food was brought from home, the container was not clearly dated and labeled in a way that identifies the owner;
Evidence
An infant cereal container located in the Infant room was not clearly dated or labeled in a way that identified the owner.

Plan of Correction: We will label the cereal container and make sure everything brought from home is clearly labeled and dated.

Description: Based on observation, contaminated or spoiled food was being served to children.
Evidence
A bottle of milk for child #1 was left out, staff stated the child arrived at 8:23am and would have the bottle between 9:45am-10:00am. Staff stated the child drinks the milk at room temperature.
In the infant refrigerator 2 bags of breast milk was observed that were dated 3/16/22-8 days old.
In the toddler classroom milk was observed in cups that had dirt spots on the top and mold in the top mechanism. The milk in the cups had a strong sour odor.

Plan of Correction: Bottles will be placed in the refrigerator as soon as the child arrives, the bottle will be warmed in the warmer to bring it to room temperature, as the parent requests it to be that temperature.
All unused breastmilk and bottles will be sent home every day.
We will wash the cups/bottles after each use. We will begin sending all cups/bottles home each day immediately.

Description: Based on observation, food is not always being prepared in a clean and sanitary manner.
Evidence
A pitcher with a top is used to hold water that is used to make infant formula bottles. The top of the pitcher has a build up a brownish substance around the rim. The substance came off when rubbed/scratched.

Plan of Correction: The top of the water pitcher may have gotten mixed up with top of the tea pitcher. The brownish substance could be a tea build up. We will clean the top and make sure the right lid is put on the right pitcher.

Description: Based on observation of bottles/cups, not all bottles/cups are being labeled with child's name and current date.
Evidence
Cups located in the toddler room were not labeled with name and current date.
Bottles located in the infant room were not labeled with name and current date.

Plan of Correction: Name and date will be put on every cup and bottle as the child arrives.
