Center management was consulted on medication authorization and allergy care plan forms. All medication authorizations and plan forms must be signed by a parent/guardian.

Today an unannounced Monitoring Inspection took place between the hours of 11:20am and 12:30pm. There were 7 classrooms observed with a total of 76 children with 12 staff within the supervision guidelines. The children were observed playing outside, eating lunch (shamrock pancakes and sausage with pears and vegetables), getting ready for nap and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medications and emergency procedures were reviewed during this inspection.

Standard #: 8VAC20-780-50-B Description: Based on record review and interview, children's records were not kept maintained and updated annually.
Evidence: The most recent documented review of Child #4's record is dated 1/21/2021Plan of Correction: We have provided the family with form to update the record. We will follow-up with the family to get the completed information.
Standard #: 8VAC20-780-60-A-8 Description: Based on record review and interview, the center has not obtained a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: Child #5, Child #6, Child #9 and Child #10 have diagnosed food allergies. The center director confirmed that they do not have allergy action plans for these children.Plan of Correction: I thought that we only needed care plans for children who had EpiPens. We will work on getting the forms from the families.
Standard #: 8VAC20-780-550-D Description: Based on record review and interview, the center did not practice two shelter-in-place drills annually.
Evidence: The centers 2021 drill log contained documentation of one shelter-in-place drill practiced in 2021. The center director confirmed that only one shelter-in-place drill was practiced in 2021.Plan of Correction: I will create a reminder on my calendar to make sure we practice two drills each year.
