Technical Assistance:
Discussed the requirement of documenting the record of daily attendance "as it occurs".

Please ensure an annual lock down drill is documented prior to the end of the school year.

Comments:
An unannounced onsite inspection was conducted today. A total of 29 children with 3 staff were present during the inspection. The children were observed arriving to the program, placing their belongings in individual bins and lining up for the bathroom. Children were observed logging onto their laptops and sitting in small groups of children at the tables. All medication onsite was reviewed, first aid kit, required postings and 5 children's records. Staff certifications were reviewed, some violations regarding the maintenance of these certificates. Staff annual training and background checks/health screenings are reviewed on an annual basis at the administrative office. Thank you for your cooperation today. Standard #: 8VAC20-780-60-A-8
Description: Based on review of child records, allergy action plans were missing for two children.
Evidence:
1. Child #1 has a diagnosed food allergy. An allergy action plan was not on file.
2. Child #3 has a diagnosed food intolerance. An allergy action plan completed by a
physician was not on file.

Plan of Correction: We will obtain allergy action plans for both these children.

Standard #: 8VAC20-780-70
Description: Based on a review of staff records, some records were incomplete.
Evidence:
Emergency contact information for Staff #2 and #3 was missing.

Plan of Correction: We need to work with the main office to obtain this information for substitutes and new staff.

Standard #: 8VAC20-780-245-J-3
Description: Based on review of staff records and staff statements, children with emergency medication were not in the care of a staff member with who has satisfactorily completed a training course developed or approved by the Department of Education in consultation with the Department of Health and the Board of Nursing.

Evidence:
1. Child #1 and #2 have emergency medication needs.
2. Staff #1, 2, and 3 did not have current MAT training.

Plan of Correction: We will work with the main office to ensure that all required certifications remain onsite for all staff to include substitutes and new staff.

Standard #: 8VAC20-780-245-L
Description: Based on review of staff records, certification that at least one staff member has current Daily Health Observation training was not found.
Evidence:
1. Staff #1, 2 and 3 were onsite during the inspection.
2. Staff #1 ,2 and 3 were unable to verify that they have had instruction in Daily Health
Observation within the past 3 years.

Plan of Correction: We will work with the main office to ensure that our documentation and certifications are onsite for all staff to include substitutes and new staff hires.
