Comments:
This inspection was conducted by licensing staff using an alternate hybrid protocol, including telephone contacts, documents review, interviews, and an on site tour of the program.

A monitoring inspection was conducted on 3/16/2022. There were no children present and 4 staff. The Inspector reviewed 4 children?s records and 4 staff records, medications, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. The toilet seat in the restroom of Class A was worn exposing the core material. Staff noted that children use this restroom when they are in the classroom.
2. The wooden border around the outdoor play equipment is rotting and splintered.

Plan of Correction: Center staff will submit a work order to maintenance.

Standard #: 8VAC20-780-330-B
Description: Based on observation and measurement it was determined the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
The fall zones for the slide and "fireman pole" did not contain the required 6" of resilient surfacing. There was less than 1" of mulch in the fall zones for the slide and "fireman pole".

Plan of Correction: Center staff will submit a work order to maintenance.
