Comments:
A monitoring inspection was conducted on 12/14/2022. There were 87 children present with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-260-A
Description: Based on documentation review, the center did not provide an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence:
1) The most recent fire inspection report available for review was dated 11/20/2020.

Plan of Correction: Complete fire inspection completed 01/14/2022.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1) On the date of inspection, areas of the center were visibly dirty and in disrepair. The inspector observed the following:
a) In the Sloth pad room (4-5-6 year olds), an used face mask was laying on a table and there were visible marks of dirt on the walls.
b) In the hallway closet in front of the teachers bathroom, there was dust accumulating on the floor along the door and corner inside the closet.
c) In the Discovery preschool room and the Toddler room, visible dirt marks were observed on the backsplash of the counters on both rooms.
d) In the playground area, in areas along the outside wall of the center: there were cobwebs, and areas of the siding that were broken, dirty or loose. A great amount of leaves was observed on the playground that almost covered the seating areas and tables. The bump pad around the basketball hoop base had slid down and was in a position were it was not covering the pole to protect from bumps. Along the wall of the hallway that connects to the preschool room, unused equipment was collecting dust, cobwebs and leaves. This is an eyesore and possible tripping hazard for the children.

Plan of Correction: Deep cleaned each classroom and hallways of center.
Work order was placed for padding on basketball pole
Removed dirty face mask.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) On the date of inspection, the inspector observed bottles of disinfectant cleaner and bleach on a lower shelf in the women's teacher bathroom. In the men's teacher bathroom, a mouse trap was observed on the floor on a corner. Both these bathrooms are located in a hallway that leads to the preschool classroom. The doors to both these bathrooms were unlocked.

Plan of Correction: Placed cleaning substances in a locked area.
