Comments:

This inspection was conducted by licensing staff using a hybrid protocol, including reviewing some records remotely and an on-site inspection. A monitoring inspection was initiated on March 3, 2022 and was completed on March 25, 2022. On March 3, 2022, there were 20 children present, ranging in ages from 4 months to 5 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies, and nutrition. A total of 3 children's records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliances with applicable standards or law and the violations were documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-780-160-A
Description:	Based on the review of staff records and discussion with the owner, two staff members did not obtain documentation of having received a negative tuberculosis (TB) screening or test within the required time. Evidence: 1. The record for Staff Member #1 contained documentation of a TB screening/test dated 12/08/2021. Date of hire for Staff Member #1 was 10/26/2021. 2. The record for Staff Member #3 contained documentation of a TB screening/test dated 03/13/2022. Date of hire for Staff Member #3 was 10/04/2021. 3. The owner verified that the TB screenings were late.
Plan of Correction:	Results of the TB tests will be on file before starting date.
Standard #:	8VAC20-780-340-D
Description:	Based on observation, staff record review and verification from the owner, the center failed to have a qualified program leader present in three different groupings of children. Evidence: 1. Staff Member #4 was caring for one child in Classroom A. The staff member did not meet program leader qualifications. 2. Staff Member #2 did not meet program leader qualifications and was in Classroom B with 7 children. 3. Classroom C had 5 children being cared for by Staff Member#1 and she did not meet program leader qualifications. 4. After looking at staff records, the owner confirmed that the staff members did not qualify as lead teachers.
Plan of Correction:	We have made all corrections; all program leaders are in the correct classrooms.
Standard #:	8VAC20-780-350-B
Description:	Based on classroom observations, review of children's records and confirmation from the director, one classroom did not have the required staff-to-children ratio of 1 staff to 5 children based on the youngest child in the room. . Evidence: 1. At the time of the inspection, Classroom B had 7 children being cared for by one staff member. Four of children were 2 years of age, 1 child was 17 months old, 1 child was 18 months old and 1 child was 21 months old. 2. The owner reported the ages of the children and confirmed that a ratio of 1 staff to 5 children was not being maintained in the classroom that had 3 toddlers in care.
Plan of Correction:	We have made all corrections to put in place a program leaser to keep the class in ratio.
