Evidence: During an interview with Administration it was reported that they can't find the record for staff #1.Plan of Correction: Ensure employee records are kept for the required amount of time.
Standard #: 8VAC20-780-550-P Description: Based on a review of records on March 22, 2022, the center did not ensure that written records of children's serious and minor injuries contained all the required information.
Evidence: The incident report for child #5 did not contain documentation of the time a parent was notified of the injury and did not contain documentation of the how the parent was notified of the injury.Plan of Correction: Ensure all staff fill out written injury records in their entirety.
