Comments:
An unannounced monitoring inspection was conducted today 3/15/2022 from 4:00pm-5:10pm. There were 33 children (school-age) directly supervised by 3 staff. The physical plant, 5 children?s records, medications, evacuation drills, and emergency supplies were inspected. Children were observed participating in group play, doing homework, coloring and doing crafts. There was an adequate supply of materials for the children. The program was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Standard #: 8VAC20-780-60-A-8
Description: Based on interviews and document review, it was determined that a written care plan was not obtained for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
Child #1 and Child #3 were listed on the Children's Allergy, Food Sensitivities or Dietary Restrictions List; their physical forms were reviewed and it was verified that they have been diagnosed with food allergies. Child #1 and #3's records did not include an allergy care plan.

Plan of Correction: The Site Supervisor will give the parents the Food Allergy Action Plan forms for each child's physician to complete.
