Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and an onsite inspection.

A subsidy health and safety (SHSI) inspection was initiated on March 28, 2022 and concluded on March 30, 2022. There were 62 children present with 18 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-600-B
Description: Based on a review of staff files, the center failed to ensure that within 90 days of employment that all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.

Evidence:
1. Staff #12's date of employment was 07/14/2021. There was not a completed Virginia Preservice Training Certificate on file. The training was required to be completed by 10/14/2021.
2. Staff verified the date of employment for Staff #12 and verified that training had not been completed.

Plan of Correction: There has been a change in administration, and current administration were not aware of the requirements. The staff member will be asked to complete the Virginia Preservice Training by the end of April 2022.

Standard #: 8VAC20-790-600-D-1
Description: Based on review of staff files, the center failed to ensure that all staff had CPR within 90 days of employment.

Evidence:
1. Staff #5's first date of employment was 11/11/2021. There was no documentation on file that Staff #5 had completed CPR certification. CPR certification was required by 02/11/2022.
2. Staff #12's first date of employment was 07/14/2021. There was no documentation on file that Staff #12 had completed CPR certification. CPR certification was required by 10/14/2022.
3. Staff verified the date of employment for Staff #5 and Staff #12, and that the CPR certification had not been completed.

Plan of Correction: There has been a change in administration, and current administration were not aware of the requirements. The staff members will be required to obtain CPR certification by the end of April 2022.

Standard #: 8VAC20-790-600-D-2
Description: Based on review of staff records, the center failed to ensure that all staff members had certification in first aid within 90 days of hire.

Evidence:
1. Staff #5's first date of employment was 11/11/2021. There was no documentation on file that Staff #5 had completed first aid certification. Certification in first aid was required by 02/11/2022.
2. Staff #12's first date of employment was 07/14/2021. There was no documentation on file that Staff #12 had completed first aid certification. Certification in first aid was required by 10/14/2022.
